An unexpected event is a starting point
An is a departure from expected behavior that requires assessment. It may originate in the spacecraft, the ground system, a data path or the expectation itself. The first useful description states what was observed and when, without prematurely turning a possible explanation into a confirmed cause.
A communication gap, for example, could be planned during an occultation, caused by ground equipment or connected with spacecraft pointing. Compare the event with the expected timeline and other evidence. Repeating an arbitrary command before understanding the state can add another change that makes the original event harder to interpret.

Protection and diagnosis have different jobs
is a mission-specific protective operating state intended to preserve essential capability. It may reduce activity while the team assesses the situation. It is not a universal guarantee of safety under every failure, and its exact behavior depends on the spacecraft design. ESA’s account of Integral describes why a changed propulsion situation required a redesigned safe-mode approach.
For a fictional overheating instrument, protection might suspend the activity according to an approved procedure while the team investigates. That action does not prove the cause was excessive observing time. Diagnosis still needs temperature histories, commands, power changes and possible sensor faults. Protecting the system buys an opportunity to reason from evidence.
Keep three questions separate
Original teaching diagram. PROTECT. What capability must be preserved?. What procedure applies now?. EXPLAIN AND RECOVER. Which evidence separates causes?. What verifies the new state?. Safe-mode behavior is specific to the spacecraft and failure conditions.

Recover through explicit conditions
A recovery plan should state what conditions must be met, what action is proposed and which evidence will establish its result. Teams may test a candidate procedure on representative systems before flight use. Controlled changes make outcomes easier to interpret than many simultaneous untracked changes.
Imagine two competing explanations: a mode-setting error and a failed sensor. A carefully chosen check that predicts different results under those explanations is more informative than simply collecting more of the same ambiguous display. The classroom principle is to connect each diagnostic step with the uncertainty it reduces, while respecting the spacecraft’s protective constraints.
